Gestational surrogacy is the procedure whereby a woman – the gestational surrogate – consents to a embryo transfer process, otherwise called an IVF Procedure. During this procedure, an embryo (or plural embryos) deriving from the couple obtaining the surrogacy service is implanted in the surrogate. If the procedure is successful, the surrogate becomes impregnated with the child of the couple (the intended parents). The level of engagement with the surrogate thereafter is completely up to the intended parents – typically, the couple is involved with the initial IVF treatments, as well as being present at the point of delivery of their child. They then assume responsibility for the child post-delivery and make the long term move to legal parenthood.

IVF is an option women choose opt for when they are having difficulties with infertility. The traditional IVF involves the removal of the human oocytes from the ovaries and the fertilization process, achieved by “letting" motile spermatozoa to fertilize the oocytes into a particular culture medium. This means that once embryos are created, they are transferred in to the uterine cavity through a process called Embryo Transfer. IVF is utilized to imitate the biological processes, in vivo.
IVF is utilized for women who have blocked or harmed fallopian tubes, thus preventing the egg and sperm from meeting naturally. IVF also may help you as well as your spouse if your infertility is unexplained and/or the male spouse has a low sperm count. Other factors like mature age endometriosis, immunology, excess weight and mindset may lead people to infertility. IVF can be the treatment for you if these factors come into play during this stressful time in your lives.
Step 1: Follicular Development and Monitoring
During a woman"s normal menstrual period, LH and FSH will be the hormones that the body uses to induce development and maturation of an individual oocyte. In order to optimize your pregnancy rates during IVF, it"s important to have more thatn one tmature oocyte for fertilization, embryo development and transfer into the uterus. The ovaries are therefore stimulated using higher amounts of the stimulating hormone that naturally exist within you. This is achieved using Gonadotrophin injections. Many different stimulation regimes are present at the moment, and the physician chooses the one suitable to your biological system, in order to achieve a good number of high quality oocytes. Close monitoring of the follicles (the sacs which contain the growing oocytes in the ovaries) is necessary and is achieved with frequent ultra-sound in order to determine their size and their amounts. At the correct timing, more hormones will be given, aiding with the ultimate maturation of the oocytes. Based on all the available opinions and laboratory results, the best time for the recovery will be determined.
Step 2: Oocyte Recovery
This is the procedure where the doctor collects the oocytes of the female body, a procedure demanding anesthesia. A small needle enters the top portion of the vagina, through the direction of the ultrasound, into the ovary"s follicles. The liquid that is aspirated will then be studied in the laboratory, where the oocytes will be identified and transferred into culture media inside an incubator at 37oC to continue the maturation process for several more hours.
Step 3: Fertilization of OOCYTS
In this step, your partner will have to give his test of sperm on the day of the oocyte recovery which will be maintained until needed). His sperm will be carefully prepared with a particular procedure and the healthy spermatozoa separately. There are about 100,000 motile spermatozoa added per oocyte, with 4-6 hours after oocyte recovery. This is in order for the fertilization to occur within the next 12-18 hours in the incubator. The insemination process will lead to the fertilization of the oocytes, and resulting embryos are transferred to different culture media to be viewed closely over the next 48 hours.
Step 4: Embryo Transfer
Good quality embryos are chosen and are transferred through the womb using a small catheter on the basis of their rate of development and morphology. The amount of embryos that"ll be transferred is set based on the age and health background of the potential mother; alongside the quantity and quality of the embryos that have been released during each therapy cycle. For every couple, there"s a possibility of experiencing more than one child. Embryo transfer is a pain-free procedure, much like having a cervical smear, and fortunately takes just a few minutes. After the task has been completed, we highly recommend a short resting period in the recovery room. One day after, oocyte retrieval progesterone and other supplements will be recommended to ensure that the lining of the uterus receives sufficient stimulation and sustains the implantation of the embryos. Quantitative pregnancy tests with blood also needs to be svheduled 12-14 days after embryo transfer.
